We provide reasonable accommodations and free auxiliary aids and services to people with disabilities so that they can communicate effectively with us — such as qualified sign-language interpreters, and written information in other formats, including large print and accessible electronic formats.
3. Language assistance services
We provide free language assistance services to people whose primary language is not English, such as qualified interpreters and information written in other languages.
4. How to request aids or services
To request auxiliary aids or services, or language assistance, please call the practice at (352) 861-2115 during office hours, Monday through Thursday, 8:00 AM to 3:30 PM. We will make reasonable efforts to provide the assistance you need.
5. This website and accessibility
Every Man’s Health is committed to making its website accessible and usable for as many people as possible. The site has been designed with readable text, clear structure, responsive layouts and attention to color contrast.

6. Filing a grievance with the practice
If you believe that Every Man’s Health has failed to provide these services, or has discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you may file a grievance with the practice.
You may file a grievance by telephone at (352) 861-2115, or by mail to Every Man’s Health, 9401 SW Highway 200, STE 502, Ocala, FL 34481-9650. If you need help filing a grievance, the practice will assist you.
7. Filing a civil rights complaint with HHS
