Erectile Dysfunction

Erections may be harder to get, harder to keep, less firm, or less reliable than they were. Whichever version of it you have, the practical problem is the same.

Erectile dysfunction is a symptom rather than a diagnosis.

Something is causing it, and what that something is determines what helps. Blood flow, metabolic factors, medications already being taken, testosterone status, other urologic conditions, nerve function, and stress can all contribute — often more than one at a time.

Erectile dysfunction can also be associated with underlying vascular disease, which is another reason to evaluate the cause rather than simply treat the symptom.

When the Pills Aren’t Enough

Sildenafil and tadalafil work well for a great many men, and Every Man’s Health prescribes them when they are appropriate. But they do not work for everyone. A failed pill is not the end of the conversation.

Often “it didn’t work” turns out to mean something more specific — the dose was never revisited, expectations did not match how the drug actually behaves, or the underlying contributor was never addressed. Those are fixable, and finding out is the first thing worth doing.

Sometimes oral medication genuinely is not enough. Every Man’s Health provides additional nonsurgical treatments for erectile dysfunction where they are appropriate. Which one suits a particular man depends on why his erections are not working and what he has already tried — which is why the evaluation comes before the treatment.

More Than an Erection

Sexual function involves more than the ability to get and maintain an erection. Desire, reliability, ejaculation and orgasm are separate aspects of sexual function and do not always change together. A man may have adequate erections and little interest, or the reverse. Both are worth raising.

None of it should be written off as age by default, or attributed to testosterone by default either. Low testosterone can reduce libido and affect sexual function, but it is not automatically the explanation for erectile dysfunction, and replacing it does not reliably fix erections.

Treatment Follows the Problem

There is no single treatment for sexual performance, because there is no single problem. What helps depends on what is actually wrong.

For one man that means getting more out of a medication he is already taking. For another it means treating testosterone deficiency that is genuinely there, addressing metabolic or vascular factors, changing a medication that is contributing, or moving to a treatment that does not depend on a pill.
