More Than a Number
Men usually arrive having already decided what the problem is. Energy has flattened, strength or drive has dropped, and testosterone is the first suspect. Sometimes that is right. Often something else is going on, and the only way to know is to look properly.
Symptoms matter, and testosterone levels matter. Neither establishes the diagnosis by itself.
Testosterone varies through the day, so it is measured in the morning and confirmed on more than one occasion rather than acted on from a single draw. And a result needs reading rather than simply reporting: a reference range describes where most of a population falls, not what is right for an individual. The same value can be reassuring in one man and worth a closer look in another, depending on his symptoms, his history and what else is going on.
What that does not mean is that symptoms alone justify treatment. A man who feels poorly with appropriately measured testosterone in the normal range has a real problem worth pursuing — it is usually just not this one.
When Treatment Makes Sense
When the evaluation supports testosterone deficiency and treatment is appropriate, Every Man’s Health prescribes testosterone replacement.
It is also not the first thing considered. A proper evaluation looks for the things that lower testosterone or produce the same symptoms and can be addressed directly — sleep, body composition, alcohol, medications, thyroid and metabolic health, stress. Some of these are reversible. Occasionally the most useful intervention is not a prescription at all.
And sometimes the answer is that testosterone is not the explanation. In that case, the next step is to find out what is.
Low testosterone can affect libido and sexual function, but testosterone replacement is not automatically a treatment for erectile dysfunction.
What Treatment Involves
When treatment is appropriate, it starts a relationship rather than settling a question. Response is judged on how you actually feel as well as on your levels, read against your own previous results rather than a single measurement. Blood counts are monitored, because testosterone can raise them. Prostate health is followed where that is appropriate.
Treatment is adjusted over time, and reconsidered or stopped when it is not accomplishing what it was started for. That last part matters. Beginning testosterone replacement is not a commitment to continuing it indefinitely.
Testosterone replacement suppresses your body’s own production and can impair fertility. For a man who wants to father children, that may make conventional testosterone replacement the wrong treatment — which is a conversation to have before starting, not after.
