Why Losing Weight Is Hard

Most men who come in have lost weight before and gained it back. That pattern is not a failure of discipline. Appetite is physiologically regulated, the body defends its weight, and age, muscle mass and previous weight loss all affect how hard it becomes. For appropriately selected patients, medical treatment changes that equation.

GLP-1-Based Medical Weight Loss

Every Man’s Health uses GLP-1-based treatment, including semaglutide and tirzepatide, when it is clinically appropriate.

These medications work with the body’s own appetite signalling. They reduce appetite and increase the sense of fullness, so the constant pressure to eat that defeats most diets is lessened rather than resisted. For appropriate patients, that can make sustained weight loss substantially easier.

GLP-1 treatment is not appropriate for everyone. Medical history and current medications are reviewed before anything is prescribed, gastrointestinal side effects are common, and treatment is introduced and adjusted with tolerability in mind.

Treatment is individual

Which medication suits a particular patient, and at what pace, depends on medical history, other medications, how treatment is tolerated, the response it produces, and what you are actually trying to achieve.

That is a clinical decision made in consultation and revisited at follow-up — not a choice made from a chart.

Lose Fat. Keep Muscle.

The number on the scale is only part of the picture. The objective is to reduce excess fat — particularly around the middle — while preserving as much muscle, strength and function as possible.

Substantial weight loss by any method includes some loss of lean mass alongside fat. It cannot be eliminated, but it can be limited, and doing so is part of the plan rather than an afterthought.

We use InBody body-composition analysis to follow changes in fat and lean mass over time, with DEXA when a more detailed assessment is useful.

Maintaining the Result

Weight management does not necessarily end when a target weight is reached. Appetite can return when medication is reduced or stopped, and weight regain is common.

For some men, continuing medication makes sense. For others, a reduced dose, discontinuation or a different strategy does. What matters is that maintenance is planned rather than improvised — decided from your own response and circumstances, and revisited as they change.
